What's on this rung

  • Sub-antimicrobial doxycycline 40mg (Oracea) — anti-inflammatory dose, Evidence A
  • Low-dose isotretinoin — off-label for refractory or early phymatous rosacea (Type 3); requires iPLEDGE enrollment and must not be used in pregnancy (severe birth-defect risk)
  • Always clinician-supervised, with monitoring

What to expect

Doxycycline 40mg reduces inflammatory lesions within 4–12 weeks without antibiotic-level dosing. Isotretinoin is reserved for resistant cases.

When to climb

For persistent vessels or established skin thickening, procedures (Rung 5) are the definitive step.
